1.   Overview

The Patient Entered Data functionality provides patients with the ability to digitally capture and send information about themselves to their providers.

With the combination of  Personal Community  and  HealthShare Care Community patients can be active participants in their Care Community care plans, including the ability to answer and complete plans sent to them by members of their care team.

The Home  page will now include a  To Do   box. The To Do box will list any outstanding care plans that have been sent to a patient, and they can simply fill out the care plan as requested, and submit it back to their care team.

All completed care plans are viewable to the patient in the  Records ,  Care  Plans   section.


2.   Patient Contributions to HealthShare Care Community

Note

HealthShare Care Community needs to be integrated to allow the ability to complete the care plan within Personal Community.

Personal Community Care Plans section   integrates with HealthShare Care Community to allow patients to enter data into a care plan. Patients will now have the ability to  be active participants in their Care Community care plans, including the ability to answer and complete plans sent to them by members of their care team.

3.   Business Decisions

When implementing Care Plans you must make decisions regarding the following two options:
  • Whether to allow patients to view only 
  • Whether to allow patients contribute to their care plans
