HealthShare Personal Community Patient Entered Data Product Area Guide
1. Overview
If there is information that an organization wants to collect from the patient prior to service, patients can receive a notification that they have a task to complete, and then complete it within Personal Community.
Patients with tasks to complete will see these tasks in their To Do box on the Home page. This box will list any pre-service tasks items such as documents, and any outstanding HealthShare Care Community care plans. Patients can access their pre-service tasks, or care plans as requested, and submit them back to their care team.
2. Patient Contributions to HealthShare Care Community
Note
HealthShare Care Community needs to be integrated to allow the ability to complete the care plan within Personal Community.
3. Event Based Tasks
Personal Community supports pre-service tasks such as allowing external systems to place educational and informational items into a patients To Do box to read prior to upcoming events.
Tasks remain in a read or unread status for a specified amount of time and if not utilized within that specified time will be moved to expired. This will allow a patients To Do box to remain manageable and current. It also gives the organization that furnishes the task an accurate account of whether or not a patient is interacting with materials sent to them.
The tasks are removed from the To Do box in the following conditions:
- All task items are completed.
- The due date for the task has passed.
- The date for the event has passed.
4. Business Decisions
With implementation of Event Based Tasks, the following information must be considered:
- What systems are allowed to send tasks to Personal Community.
- What are the triggering events in those systems to send the task.
- What is the content that will be sent.
- Whether to allow patients to view only
- Whether to allow patients to contribute to their care plans
