HealthShare Personal Community Patient Entered Data Product Area Guide
1. Overview
Personal Community allows sites to send digital packages to patients as tasks within Personal Community. These packages can consist of educational materials, instructions, Personal Community digital forms, or requests for patients to upload physical documentation (such as pictures of insurance cards). Here, patients are able to review provided materials and complete provided forms or upload documentation requests prior to coming in for services. This functionality can also be extended to provide patients with a digital discharge packet that a patient can review after they have gone home.
In addition, with the combination of Personal Community and HealthShare Care Community patients become an integral member of the care team. This combination allows patients to be active participants in their Care Community care plans, including the ability to answer and complete plans sent to them by members of their care team.
If a patient has a care plan or task to complete, the Home page will now include a To Do box. This box will list any outstanding care plans or task items that have been sent to a patient. From here, the patient can simply complete the requested to do items; completed digital forms can become available as needed, uploaded documentation can become available where needed, and completed care plans will become available within Care Community.
2. Patient Contributions to HealthShare Care Community

HealthShare Care Community needs to be integrated to allow the ability to complete the care plan within Personal Community.
3. Patient Tasks
Personal Community supports pre-service tasks such as allowing external systems to place digital forms, educational and informational items, or requests for documentation into a patient's To Do box to review and complete prior to upcoming events.
Tasks remain in a complete or incomplete status for a specified amount of time and if not utilized within that specified time will be moved to expired. This will allow a patient's To Do box to remain manageable and current.
The tasks are removed from the To Do box in the following conditions:
- All task items are completed.
- The due date for the task has passed.
- The date for the event has passed.
Organizations have the opportunity to receive updates on the status of the tasks that have been sent to patients. This provides the organization that furnishes the task an accurate account of whether or not a patient is interacting with materials sent to them.
4. Business Decisions
- Whether to allow patients to view only
- Whether to allow patients to contribute to their care plans
With implementation of Patient Tasks, the following information must be considered:
- What systems are allowed to send tasks to Personal Community.
- What are the triggering events in those systems to send the task.
- What is the content that will be sent.
With implementation of status updates for patient tasks, you must make decisions regarding the following:
- What systems will receive status updates from Personal Community
- Which status updates will be sent to the systems
- How the status updates are used in those systems
