Custom Schema Creation
Custom Schemas can be created using the HL7 v2.x Schema Structures page. Here are the basic steps:

Identify a base schema. Most custom schemas involve changes to a relatively small number of message types from a standard schema. Message types that are not explicitly customized will be defined by the base schema definition.

Copy the message types that will change from the base schema to the new custom schema.

Create some new segments or other elements for the new schema.

Customize the relevant Message Types and DocType Structures (also known as message structures) in the custom schema.
Note:
Prior to the 2013.1 release, custom schemas could only be created using the Studio development tool. They can still be created using this tool.
